HCS Discharge Summaries now live across Tasmania

HCS is pleased to announce that HCS eCCO, our integrated electronic discharge summary solution, is now fully operational at three public hospitals across Tasmania.  HCS eCCO was initially implemented by the Tasmanian Department of Health & Human Services (DHHS), as part of a patient discharge medication record pilot scheme at the Launceston General Hospital (LGH). 

Following the success of the pilot program, DHHS contracted HCS to implement the solution into another two Tasmanian sites, Mersey Community Hospital and North West Regional Hospital. Since the go live for the additional 2 sites, over 9,000 discharge summaries have been sent electronically to GPs. This has assisted in ensuring patients receive the highest quality of care post discharge, subsequently reducing the rate of adverse medication reactions and decreasing the occurrence of re-admission to hospital.

Since implementation earlier this year, HCS eCCO has received positive feedback from treating physicians and GPs alike.  The success of the product is highlighted by comments from system users and GPs which reflects their satisfaction with the new discharge summary creation & delivery processes. 

HCS eCCO is part of the HCS Clinical Suite, elements of which are also implemented into Tasmanian DHHS sites.  Designed to automatically draw information from a range of clinical systems, including HCS PharmCare, our electronic medication management solution, HCS eCCO enables multi-disciplinary contributions to the discharge summaries.

HCS PharmCare set to help transform SA Clinical Pharmacy Support Services

The implementation of HCS PharmCare has been touted as a source of much needed support for South Australia’s Clinical Pharmacists.

“The implementation of careconnect.sa clinical pharmacy support (using Pharmcare) has the potential to be a step forward in standardising and supporting clinical pharmacy activities, to improve continuity of care in line with the APAC national standards” stated Clinical Pharmacy Services, Acting Team Leader, Olimpia Nigro. 

Designed specifically to support the practices of Clinical Pharmacists in an acute care hospital, HCS PharmCare encompasses all elements associated with the provision of patient care, from admission interviews, to decision support, drug interactions and adverse drug reaction alerts, to the creation of  patient medication counseling sheets for post discharge support.     

Implementation of HCS PharmCare into 12 South Australian hospitals will commence in the New Year, with the first site expected to go live in April 2010.

Merry Christmas from HealthCare Software

The HealthCare Software team will be having a short break over the holiday period to enjoy the busy Tasmanian Festival season (www.discovertasmania.com.au).  The HealthCare Software office will be unattended from Thursday 24th December, 2009 to Monday 4Th January, 2010, when we’ll be back to business again.  Our normal after hours support will be available during this period, for any unforeseen issues which may arise.
