PharmCare to roll out across Tasmania 29 October 2007
The Patient Discharge Medication Record (PDMR) project, which involved the use of PharmCare to deliver discharge summaries from the Launceston General Hospital (LGH) to GPs, has now received funding from the Department of Health and Ageing for a Tasmania-wide implementation. The PDMR project was successfully rolled out in the LGH through an ongoing partnership with HealthConnect Tasmania and has significantly improved clinical activities.
HealthConnect director Mary Blackwood said HealthConnect remains on line to deliver improved services to the Tasmanian health sector through its program of funded eHealth projects. "The permanent benefits of trials like the electronic Patient Care Record (ePCR) and the Patient Discharge Medication Record (PDMR) will be continued in the eHealth work that we are funding around Tasmania in this new period."
The Tasmania Minister for Health and Human Services, the Hon Lara Giddings MHA, has previously said of the project "Effective communication is absolutely crucial - especially when it comes to our health. Poor or slow communication between healthcare professionals is a problem we all want to fix. The PDMR initiative goes a long way towards that - using the latest technology to improve how healthcare providers 'talk' to each other."
Healthcare Software director John Elkerton said "the roll out of the PharmCare discharge summary solution in Tasmania represents a major step in the improvement of care for the people of Tasmania. It is also a testament to the hard work from both the HealthConnect Tasmania team and Healthcare Software that the pilot project has advanced to this implementation stage."

Healthcare Software also extends its congratulations to HealthConnect Tasmania for its achievements at the Project Management Achievement Awards. The annual awards, organised by the Australian Institute of Project Management (AIPM), recognise best practice and innovation in project management. The purpose of the Institute is to encourage excellence through professionalism in project management. HealthConnect Tasmania received two awards; Organisational Change category winner and Overall State Winner.
